How to Register as a New Patient

Registering with the surgery

To register with the surgery please visit reception and collect a paper registration and Health Questionnaire form.

Appointments are not always necessary to register with the practice. During your registration you will be required to have a new patient check which will enable your registration to be fully complete. You are advised to bring any medication you are taking so this can be entered on your records for future prescriptions.

If you are registering a child and are from outside of the UK please provide all copies of immunisations/vaccine records you have to ensure us to keep your child's medical records up to date or for us to help you book your child with the nurse for any missed vaccinations.

Before filling out a registration form, check you are in our catchment/boundary area. If you are outside of the of the Practice boundary area you can visit the following link to Find out which GPs you can register with on the NHS website.

Please note that NHS England only allows a patient to register with one practice. This means that you will be de-registered from your current GP practice.

Should the service not work for you, we will support you in transferring back to your previous practice.

When you register, it’s helpful to have your NHS number. If you've had a GP in England before, please locate your NHS number as it will speed up your registration process. Here are four ways to find it:

  • on NHS letters
  • on prescription papers
  • on the NHS app or on a child's baby red book
  • You can use the NHS website to find your NHS number.

If you move to an address outside of our practice boundary, you will need to contact reception & enquire about remaining with the practice as an out of area patient.

Temporary Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Named Accountable GP

From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.

Your named accountable GP is Dr ST Kumar.

The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.

  • Patients do not need to see their named GP when they book an appointment with the practice.
  • Patients are entitled to choose to see any GP or nurse in the practice.
